The Centers for Medicare & Medicaid Services (CMS) in 2016 began reimbursing for advance care planning (ACP) to physicians as a payable service for traditional Medicare beneficiaries. Advanced Care Planning is the face-to-face time a physician or other qualified healthcare professional spends with a patient, family member, or surrogate to explain and discuss advance directives.
The ACP program includes the explanation and discussion of advance care directives and the use of standard forms furnished by physicians during the face-to-face meeting discussions with beneficiaries, their families, and surrogates. The discussion often focuses on end-of-life issues and decisions that involve when beneficiaries are incapacitated and cannot make certain medical decisions.
